Pizza delivery shop question

So I'm doing that pizza delivery shop where you have to take 6 photos and turn some slices upside down. Ok there may be more than one like that, this is my first. So first of all, worst of the 20 or so shops I've done so far from a customer service respect. The guy on the phone was very terse, very monotone, and sounded like he wanted to be doing anything but taking pizza orders over the phone. No big deal. But then 30 minutes later the delivery driver calls me to let me know he's here with my pizza. I live on a hill, driveway at the end of my house dead ends into what used to be a garage and is now closed in with no door. I'm in the process of turning it back into a garage, but right now it's just a big room full of boxes, tools, etc. 10 yards from my driveway is a set of steps going from the street to my front door. So delivery driver calls, obviously kind of perturbed that he has to call me, and not exactly rude about it, but close to it. What do I put on the shop? If I describe that I'll no longer be anonymous, but if I don't I feel like I'm not reporting the truth. The driver wasn't very smart to begin with, don't pizza drivers ALWAYS deliver to the front door? And then he was almost kinda rude about it.

Pizza drivers are usually under pressure to deliver as quickly as they can, not only because they are expected to deliver a lot of deliveries and be back quickly to take more, but also because the customers want their pizzas as quickly as possible. The driver was probably frustrated because he could not see your door and was not sure where to go to deliver to you. If a driver doesn't see your door, frequently he will telephone rather than jump out of the car and walk around. I live in a small, one-lane private street with four houses. New pizza drivers almost always telephone me when they pull into the street because they are not sure which house I am, especially at night. I usually answer with a description of which home is mine (and usually I can look out my window and see them) and I give directions such as "Park in the circular driveway and come up the sidewalk to the front door, up the stairs." They always gratefully thank me and bring the pizza to the front door. I have never had one ask me to walk out to their car to get it.

What to write in the report?

The truth of course. How much of the truth is what you need to consider to remain anonymous. Look at the report and answer the questions accurately. If the report is the same as the one I'm familiar with, you shouldn't have to describe your house or location, but definitely mention more facts (or absence of required courtesies) than listed above. For example, did they say hello or good evening? Introduce themselves? Say thank you for calling? How can I help you? Or perhaps they opened the call by saying something like, "Pizza delivery! Where are you dude?" Those details or omissions will let the reader see how friendly or rude the staff was. If there is a place for your opinion or how the experience could be improved - there you can state how it would have been friendlier to do things in a different way.

I know for a fact "John" has your account pegged with the words shopper anyway. So your annoymity is already gone. Had a friend of a friend see it on several accounts and it was brought up in conversation. I do not do them any longer. Not only that what if you give them a flunk? They have your address. And employees come and go. What if you were the cause of an employee let go? Just a food for thought.
