Maritz evaluation form help needed!

Hi. I just did a bunch bank assignments. I thought I entered the correct APPROVED document (one of them) but I just got replied the one I upload is wrong. Do I submit a copy of the front of the brochure? I thought any of the samples listed are approved. Help please. Thank you!

I always upload the first page (or cover, if it is a booklet) and have never had a problem. For the ODP ones, I upload a copy of the inside of the opened brochure. Do you know which one they said is wrong?

I upload the page with title "Additional Banking Services and Fees for Personal Accounts"...which is on the approved list of documents.

Why don't you give them a call? It may end up saving you some time if they can identify the problem so you be sure you are uploading the correct document.

Thanks. I just called and was able to speak to someone.

I was told the best document to submit would be the cover. Although I submitted "Additional Banking Services and Fees for Personal Accounts"...it was marked wrong because it does not have the heading "DEPOSIT ACCOUNT AGREEMENT" in front of the title.
