The only remaining BP in my area has a manager who insists on walking the entire property with me every second and he gets totally paranoid if he sees me writing on the form or taking photos. One time he grabbed my paperwork and wouldn't let me leave until he'd screamed at an employee to fix whatever before letting me get out of there. I whipped out my phone to call the cops and he tried to grab it out of my hand. Things got ugly after that.

I don't mind a lot of photos. I've become super efficient with them. What I hate is how disorganized the layout is. In the store, out of the store, in the store, out of the store. Questions are numbered 9, 12, 3, 17, 18, 6. You can't find anything in the book. Then there's that extra page.
There's just no flow.

I wonder what kind of fine or punishment they get if there are bad things wrong?

I have never cared for these shops either. It's not because of the signatures - It''s the paper reports themselves.
I'm not certain what they are for since they are not even sent in and the signed report cannot be reviewed by the onsite auditor (you) anyway. So what is the signature for?
The proof of your visit is in the receipts you receive as a shopper. The reports themselves are a problem since the shopper has to await the FedEx shipment before performing the shop.
I suppose if the attendant hesitates to sign the paper report, it would be easier to just say "okay" and note that they refused to sign the report in the comment section of the online report.

That's exactly how I feel about Papa John's.
