If you answer NO, they did not offer an upgrade: What more is there to say in comments?

I just completed a shop that asks Yes or No, did the worker offer
An upgrade
A sample
A complimentary product
Greet me

This person did none of these, No greeting, no help, just the average lalalander

So, what are they looking for? A few sentences?

No, said person did not offer an upgrade ........and then what?

Most companies want a comment for all "no" answers as confirmation, so I would write, "The worker did not greet me. They did not offer any samples or complimentary products. I ordered the eensy-weensy xyz and they did not offer an upgrade."

They are looking for confirmation of your NO answer. They want to be sure you meant to check no and that it was not a mistake. A little more information as well is helpful. In many cases, when the associate does not do something that is expected , like greeting you, offering an upgrade or sample, or suggesting a complementary product, he has "failed" part of his performance. Many clients address these "failures" in a performance review. A little more information than "no" is usually helpful. Like: "Although Susan smiled when I approached her, she did not make eye contact and she did not greet me. Susan showed me products that met my stated needs, but she did not offer any samples." Or, "After taking my order, Susan asked if I wanted anything else, although she did not specifically suggest anything. Susan repeated my beverage order, but she did not suggest that I upgrade to a larger size. Although Susan demonstrated the product I asked her about, she did not mention any complementary products."

I try to give the associate credit for what they DID do while making it very plain what they did NOT do.
I see, thank you very much! When you wrote out the examples, I could see why they would want this in written form. I'll need to remember to account for that time, also, when I'm accepting shops. It was a fun shop.
I try to address every question (whether answered yes or no) in my narrative. That way there's no question if I forgot something.

I always write a sentence for all "np" responses. Its the MSC way of double checking your responses. My reports are not returned which mean it moves on to payment.
