I've had two rejected, and honestly I just sucked it up, didn't bother getting clarification because I knew why, but they called about two days after on each, and told me that my report was reinstated and was now back to "In-Process".
The problem with mine was that the store I was assigning myself to, they are very customer-friendly. So if you call in placing a one-topping order, and there's a current coupon code out for a better deal for a two-topping, they will ring it up as a two-topping, even if it's made as a one-topping. Make sense?

To be totally honest, I *think* it went to "Needs Clarification" before getting rejected, and I just didn't check often enough to catch it. I consider myself lucky, (and them awesome) that they then called me and re-instated it.
