Apartment shopping notes form

I'd like to make a sheet for apartment shop notes. I'll use this form while calling and then when I get back into my car after a shop. (I pull over up the street and jot notes.) Any ideas on what I should add? If I'm going to take the time to make a form, I'd like to try to get it all on the first try.

So far the areas I have are:
Apartment name
Address
Due Date
Pay Rate
Mileage
Personal info given
Person shopped
Shop date/time
Description of person
Length of shop
Bathroom location
Lobby description

Personally, I refer to the floor plan provided by the agent or the community’s website to describe the location of the bathrooms. Very helpful when I’m shown two or three different layouts.
The form I use is pretty much the same, although I break out the agent’s description by height, weight, age, hair color, eye color and outfit. I recommend having a line for “Name tag (Y/N)”.
Each MSC does have different questions that must be answered.... I will often get into my car after completing a shop, turn on my voice recorder, and rattle off answers to all the MSC's specific questions. I find that, when I do this, I generally remember the answers when writing my reports and don't even have to refer to the recording. And, when I do forget, I have the recording.... It really speeds up the process of writing long reports.

I always forget the leasing consultant's eye color and the apartment number. And I just refuse to guess someone's weight.

I’ve never been able to guess people’s weight. I provide a range like 120 to 160 pounds, and even that comes with a huge margin for error.
The company that asks for eye color no longer asks for apartment number.
If you cannot remember:
Females are 5'5", Asians a little shorter
Eyes are always brown; you will notice if they are blue.
If you remember their height, look up the weight for the body type on Google.
Since you generally ask the shopper-revealing Fair Housing questions at the end, you should be able to speak their answers into you recorder or smartphone as soon as you return to your car.
Follow MF Johnston's advice and either pull over and fill out the written form immediately, or talk into the recorder and speak the answers to each question on the form. That is faster and you will be able to record more information before you forget details.
Use the note-taking app on your phone as long as you set it to airplane mode before you started your shop ( you did turn it off didn't you) you can simulate making a phone call as you leave the office. Hello Betty glad I caught you let me tell you about the ugly apartment that I just looked at.
Mostly just to verify that you were actually in the apartment. 90% of the time, I’m uploading a picture of the floor plan. It’s kinda silly. I write “The bathroom was in the hall between the bedrooms” or “The master bath was in the master bedroom”. Never had an editor mention it.
It all depends on the model shopped. One bedroom, Jr one bedroom, Townhouse with or without garage two or three bedrooms.
I've done a lot of apt. shops & I was never asked for the location of any of the rooms! They ask for amenities & rent & why they are different at certain times.
Then you have not done shops for the company they are referring to. The company has lots of apartment shops.
