You will call one of the locations listed to set up an appointment with the provider (doctor) listed and go to the visit with your child for a wellness appointment (check up). Write up about your experience calling in and on a second report write up your experience of the in-person visit answering specific questions provided to you prior to the assignment.
Scope
For this study, mothers, fathers or grandparents with their legal children under the age of 17 will make a new patient appointment at one of the three locations identified for this study and bring in their child for a wellness checkup, sports checkup or an actual sick child appointment.
Prior to the appointment the shopper will call the location to secure an appointment and write up a report detailing the telephone experience. Fill out the form online.
Each contracted mystery shopper signs a confidentiality agreement written by our corporate attorney.
Shoppers will be paid $50 for the call and visit combined. The visit is free. We need to know the shoppers name and exact date of appointment to set up payment of the wellness exam for their child ($150 value) The shopper will say they are paying cash when setting up the appointment and arrangements will be made at the time of the appointment to either put a credit in their account or they will ask for a prepaid credit card upon checking in and use it to pay when checking out.
It is important that the shopper review the questions on the survey prior to going in for the appointment. (provided once they sign up and fill out our forms)
They need to write down in their iPhone or paper the names of the frontline, nurse, physician (each assignment is for one visit per physician mentioned for the study) and the checkout person. The details of each encounter and how they and their child were treated is important.
