Has anyone done a credit card shop where it gets to the point of them showing you a consent form?

The scheduler told me to use a fake name. At what point in the conversation with the banker are you given a consent form? Before or after you show them an I.D.?
I have done credit card shops for other MSC. None of them got to the point of being shown a consent form.

I talked to the scheduler. I was told to act interested and say "Yes you would like a card" I have a feeling once you say this, they will ask for an ID. Yet they want you do use a fake name. I might try it again.
I have done several of them 1. It is not legally binding until you sign. 2. You have to be provided a terms of services copy it is your right as a customer 3. From what I know you usually need to provide ID when starting a CC application due to fraud reasons.

I am not sure how you can apply without providing ID, if we didnt need ID imagine the fraud.
I completed this shop one week ago. I also spoke to the scheduler about the ambiguity in the instructions. I too have completed multiple credit card inquiry shops and have never been asked to sign a consent form. I asked to know specifically when a consent form would be introduced into the conversation. That question was not answered. I was told I could stop the shop when a consent form or social security number was requested. I was still a little uncomfortable.

I did the version of the shop where I had to be an existing member. I told the banker about my interest in a new credit card. She walked me through each of the available cards which fit my needs. She made a recommendation and asked if I was interested in applying for it. I said yes. She asked to see my bank card. I said I left it at home. She asked for my ID. I apologized and stated I wanted to discuss the recommended card with my partner first. A consent form was not introduced at any point in the discussion. I stopped the shop because I did not know what would happen once I handed her my ID or credit card. I was concerned she would run my social security number after I presented either one. The ambiguity of the instructions really left me with little room to do anything else.

I submitted the shop. It was accepted and graded.
Is this for the blue bank? I haven't shopped them in years, but I do know that they now allow people to check for any pre-approved overs by entering their name and address on their website. I can't remember if this also includes birthday and last four of social. This is not a hard pull. Is the consent form basically permission for the employee to do this at the branch?
