This has been discussed recently. The answer is PV-500, with a special button camera.
For more details, just do a "search" on "video" and set the period to "last 90 days."

Expect to spend $350 to $400 for a new PV-500 rig.

You want a PV-500 with a button camera. This is the industry standard and the only acceptable device for many MSC’s.

There are several distributors. I bought my last rig from Greyhawk Video Solutions. Their website is easy to use and has a Mystery Shopper section.
