I got a text message from "nctraining". I did not click on the link, but typed the name into my search bar. Came up with a page that said down at the bottom all info collected on the form belongs to the form's owner, and will be forwarded to the form's owner. There's no real website.

Unfortunately, this type of email is being sent to random people who do not actually do MS. And since people are in financial stress, many of them will fall for it. You should warn your friends and family members.
