Audio: I keep my phone in my hand.
Video: The phone is the WRONG piece of equipment. You want a PV-500.

Even when the MSC allows phones for video, there is always a better option.

Pv500 all the way for video. If I can't use the video recorder, I've been known to hide the phone in the bra which keeps it high enough to capture conversation without background noise and without being accidentally covered or turned off.
I put my smartphone in my shirt pocket when not using the PV-500 to record. When I use the PV-500, I put the DVR in my belt clip under my shirt, and the button camera goes on my chest in my shirt's second button hole from the top. I pin the mic to the inside of my shirt pocket.
Two years ago, I completed a video job where the MSC initially declined to accept the shop, this due to MY voice over modulating; it was still easily heard. I responded that my volume was high to insure a successful capture of the target; they agreed and the job was paid. I mention this work to point out that placement is but one consideration for success.
Audio, I use a voice recorder and keep it in a small embroided bag that hangs on a long shoulder string. Luckily I look like a past my prime hippy to most people anyway so it works for me. It's my hair folks. Out of control curls since I was born. If seated I just set it on a table.
And a warning about using a bra to hold anything. Please try it first if you are someone who has enough to fill that cup as you will find that bending and twisting pushes buttons on your device.

I use a sports bra type that works well for me but I agree, try it and make sure it works before you use it on the shop!
I have used a tablet to make recordings of some drive-thru Chicken shops. I had to wedge it into the seat where you could barely see me. I have taken videos with a cell phone one sucked so bad. Then I was at a store and bought a new holster for the cell phone, and I realized that the foam that fills the holster out on the shelf would work as a backstop to keep your body from pressing the screen and changing the recordings. That only applies to males that have to take 10 mg of Prednisone a day.
