You are required to complete a pre-visit recorded call and book an appointment. When you call, you
will indicate that you would like to book a new patient appointment. The phone greeter should inquire as to the reason for
your visit. Please indicate you have recently moved to the area or haven’t been to the dentist in a while and would like to
get established as a new patient. You will only be reimbursed for a New Patient Offer which includes: dental exam, x-rays,
and BASIC cleaning. Most locations will only offer an exam and x-rays on the first visit. Sometimes the cleaning will be
booked on a 2nd appointment. If this is the case, you may schedule this cleaning; however, you will only report on your first
initial visit. If it is not mentioned, you will evaluate the exam and x-rays only. Additionally, no other services, outside of the
New Patient Appointment, will be reimbursed. Fluoride is an upcharge for Adult visits and will not be reimbursable.
