*This shop requires a dental exam and x-rays. You will ONLY complete a cleaning if is offered as a standard part of the new patient appointment.
*Record your PRE-VISIT call to the location via our system to make the appointment.
*You MUST be 21 years of age to conduct this shop.
*You will be reimbursed for a set of dental x-rays, a regular exam, and a routine BASIC cleaning.
