GUIDELINES:
You are required to call ahead to place your order.
This call MUST be recorded using the toll free number IN THE SURVEY FORM.
After ordering your pizza, go to the Location and pick up your order

A Medium Hand-Tossed
Round Pizza
of YOUR Choice
Photos:
BEFORE eating your food items
Remove the pizza from the box - leave the pizza on the white liner
Take one picture at a 90 degree angle (directly above the pizza)
Receipt
Upload your receipt to the report (on the website) and you will be reimbursed for the price of a medium hand-tossed pizza.
