Pre-paid credit card shops

Hello everyone, I will like to do the pre-paid credit card shop from Serviceintelligence, but when I went to do the certification for this client, the tips stated that you need to fill up a form and if asked give your Social Security number and when the employee try to input the information in the computer you have to stop the sale and finish the shop.
I am not sure about this shop, what happen with the form ? They return the form to you ?.
If any of the fellows shoppers did this shop, I will like some information, because I don't like give any personal information when I do the shops.
