The forms we would have access to would be considered by the MSPs we work with to be their confidential, proprietary materials, so I don't think anyone here can accommodate you. I doubt seriously that the MSPs would offer you a form either. Your best bet would be to just create one yourself: date of visit; time of visit; location; pre-visit phone call results; who assisted you on site; etc. It would then go into what they showed you and what they told you.
