I would suggest not looking at the menu. That makes them think you want something else or are still deciding. Look them straight in the face or stand there with your card in your hand waiting on a total. That gets them moving.
You basically need to nonverbally indicate that you're done.

You're right in that it makes sense to logically have a drink of some sort when ordering a meal. It can be dry without one. I usually get a cup of water if dining in.

Actually both items are weird to NOT order. Now, I'm even more paranoid!!!
eta: Also, how do you respond when they offer the extra item(s)? Do you pretend to think about it? I usually do. I'm like: "Ummmm.....suuurrre, I'll take a small drink." My tone tries to act like I'm really struggling to decide.

I feel if I were to eat out somewhere that I wouldn't just forget to order a big item. And, if I didn't want any to begin with, then I wouldn't suddenly be like: OH SURE, I'LL TAKE THAT GIANT ORDER OF FRIES! just from someone suggesting it.......
When I am not mystery shopping, I enter every restaurant with the plan to skip the fries, then, more often than not accept them once offered. So, all my life, I have ordered burgers only, then got fries when suggested. On my own I say no to pop because I dislike it.
