Mystery Shop Questions

Can someone give me sample of evaluation or mystery shop forms and categories for evaluation? thanks.

That is a difficult (impossible) quesion. You need to apply and read instructions on the job, way to many variations for us to give. Seems like you want to know questions to assignment before accepting job, we would not know what job you are asking about, it just doesn't work that way.

And of course, even if we knew which shop, our Independent Contractor Agreement would not allow sharing more than just the most general of information about the shop.

Generically, most shops need the time of your visit, they have a specific set of observations for you to make which may include: cleanliness, how well stocked the location appears to be, whether the place looks well maintained, when and how you are greeted, how employees responded to your request, names and descriptions of employees with whom you interact, experience at checkout, condition of restroom. Each shop will have some twist or variant on this generic list.

That's what I needed to know. I am interested in the kind of questions for a retail pharmacy setting. I recently started working with a pharmacy management firm and they too want to evaluate their clients locations.
I know some of them grade them too. Any ideas?
In a case like that, it would be important that the pharmacist or technician sticks to standards when verifying a customer's identity, too.
I gather you are attempting to put together surveys, which is even more reason why it would be a violation of our ICA agreements to give you the questions that others ask. Theirs are proprietary information which they have spent the time and energy to develop.

Let me suggest that you start with the client and find out what THEY want to know? Most any business is bombarded with "best practices" comments from their trade assoc. and regulators.

You might contact a mystery shopping company. They may be able to provide you with a sample evaluation to try to get your business.
