Mystery Shopping form

I need some ideas to create a evaluation anonymous survey form I can use when I am visting restaurants. Is there such a form available that can be used and modified?

Are you talking about something such as a shopper would complete after a shop or a cheat as a shopper to take with you or just what.

Our Independent Contractor Agreements prohibit us sharing any survey that we use with a client.

Perhaps a little more explanation of what you are trying to do would give a little more opportunity to assist.

Every MSP has a different evaluation form for their clients. They base the evaluation on what the clients want evaluated so if you are asking if there is a basic outline of form that you can add/delete stuff........I would say NO.
You will have to use the form that the MSP company provides you with when you are assigned a restaurant shop. Hope that helps!!
