I try to give them the facts also, however this is the post I was responding to.
"NEW in 2011! STN will be conducting Note Audits to ensure accurate information is recorded. You MUST keep your notes for 30 days after your visit as you may be asked to send them in to STN for verification of the information in your report."
When I fill out the report I report the facts as I see them and IMO that information pretty much completes my obligation for that shop. Requesting my personal notes in any form is borderline stepping over the line. I would find it especially onerous if they said just send in all your notes without stating what they were looking for. I believe it's my choice how many notes I take or don't take and what specific way I take them. That is why I stated that if they want something specific they should make a downloadable "notes" form and say you must use this form when transcribing your notes. It's like they want a another report to to verify the report.
