It could be. After my husband died last year I lost every single one of my B of A deals and had to start from scratch even though it was a joint card. They told me it is based on your spending patterns. I had to slowly build up to getting 25 or more deals each period.
Most of the time I never even heard of half the deals I get on Am Ex. but I did use a $20 off $100 once when shopping at a high end restaurant! The rebate is not reflected on what you get billed for your meal. You get it afterwards as a credit on your credit card.

Most of the shops in my area DO have the milkshake requirement, but only over the last year have these locations added milkshakes to their menu.

I started on these little avoidance s of restaurant charges when my kids were babies and I would bring their sippy cups along to restaurants to control what they drank when the restaurant did not offer something I thought healthy for a baby. Then when no one questioned me I continued to bring it along until they were too old and put milk in it instead of ordering a glass for $2-4. Also would get the xtra large fries at fast food to share rather than costlier indiv portions. And now I bring my own washable plastic ware for leftovers when I eat out to save the environment one throwaway container at a time. Label me cheap or thrifty but my financial situation is much better due to some of these small savings over time.
We all make newbie errors. Next time you do a Five Guys, pay close attention to the drink requirement. It cannot be bottled water for example. It must be iced tea or soda. The assignment will state specifically if a milkshake is required. In either case, you still have to get the soda/iced tea.
