I had a job kicked back to me for "not following instructions," but I followed what was on the guidelines. When I asked the editor for clarification, I just got a "do what I said" reply, with no clarification on what I was supposed to do--which still wasn't clear. I muddled through it, and it was accepted. But I could just as easily have done the re-do wrong and had the report rejected.
You try to do a job right and try to be pro-active in making sure the instructions are clear, but sometimes it's not appreciated.
I learn something new every day, but not everyday!
I've learned to never trust spell-check or my phone's auto-fill feature.
